This amputation specimen is from a patient with diabetes mellitus and severe atherosclerosis. This is gangrene. Poor circulation leads to necrosis of the soft tissues as well as propensity for secondary infection. There is such poor healing that the gangrenous areas must be surgically excised. Here can be seen black infarcted areas of toe and heel. Above the ankle is an ulcerated area of skin extending to soft tissue.
