The multiple small nodular hemorrhages seen here on the hand (Janeway lesions) resulted from peripheral embolization of fragments of a left-sided valvular vegetation in a patient with infective endocarditis. The irregular reddish purple mottled areas of skin represent "livedo reticularis" from ischemia following embolization to medium-sized arteries.
