It is unusual to see neoplasms get to this size, but this patient just didn't consider seeing a doctor because he had no regular job with a health care plan. This is a squamous cell carcinoma. It actually arose in a draining sinus tract from chronic osteomyelitis of the humerus (following a gunshot wound). There is a significant segment of society whose problems seem always to be beyond the scope of optimal medical treatment.
